How did you get my information?

Patient Outreach obtained your information from a contracted entity (e.g. Hospital, County, State Agency, etc.) for the purposes of evaluating and assisting you with your health conditions. As required by Federal and State law, any information shared will be kept confidential and only used for the limited purposes under contract.

Are you a collection agency?

The covered entity is authorized to share information with Patient Outreach. The entity and Patient Outreach are business associates under Federal and State law, meaning that any information shared will be kept confidential and only used for the limited purposes under contract.
